We will adhere to your insurance company guidelines in order to provide the best possible care for your injury. We will need your assistance in order to comply with possible insurance requirements that might affect your policy. Please provide a current insurance card so that our office can keep a copy on file.
FOR INSURANCE PLANS THAT REQUIRE REFERRALS OR AUTHORIZATION: You must provide either at the time of your office visit. Otherwise, you will be expected to reschedule your appointment. This is your insurance requirement and, therefore, your responsibility. Please call your insurance company for plan details. If you multiple insurance plans, you may need multiple referrals and/or authorizations.
Please remember that insurance is considered a method of assisting the patient with medical expenses and is not a substitute for payment. Payment is expected at the time of service. We collect co-payment at the time of visit. For your convenience we accept cash, check and major credit cards.
WORKERS' COMPENSATION INSURANCE: The office accepts workers' compensation insurance, with the approval from your representative at the insurance company handling your claim. Please contact your claims adjuster in order to get an approved office visit. Our office does not participate with all networks. AUTO INSURANCE: The office accepts auto insurance. Please bring your Auto Insurance Card to every visit. Our office can only verify auto insurance coverage at the time of visit, and the patient will be responsible for copays and exhausted benefits.
